Service Description
Sinai Chicago is committed to identify and develop innovative, evidence-based solutions to gaps in service and community issues, such as trauma and violence, and raise community awareness related to behavioral health and its impact on overall health. The integrated delivery model aims to restore and enhance the quality of life and reduce overall costs of healthcare for the populations we serve. We provide application assistance to uninsured individuals seeking services.
We offer case management, psychiatric medication management, substance use screening, psychiatric assessments, medication-assisted treatment, and crisis evaluation. The goal of our Adult Intensive Outpatient program (I.O.P) here is to enhance a patient’s mental health and allow the patient to live their life as normally and productively as possible, as soon as possible. Services and activities are tailored to each patient’s individualized treatment plan goals, identified needs, and preferences for treatment and include meals and transportation.
