Service Description
At PrimeCareHealth, your care team will coordinate the care you and your family receive in our centers. Case managers will help assess your health, coordinate care and work closely with home health agencies and community-based agencies to assist you with important services like transportation, housing, utilities, food and managing your daily life.

At all sites, behavioral health consultants work closely with primary care providers to support patients in making lifestyle changes necessary to attain and maintain good health. Services include programs to effectively manage emotional or behavioral difficulties such as anger, anxiety, depression, stress and bereavement. There are also support services for behavioral change plans related to smoking cessation, weight loss, alcohol use, exercise and other lifestyle modifications.
